Payments: The rate you receive is a commitment for the entire school year. If you decide not to continue for any reason, you will still be liable for payment, or the rate will increase for the remaining participants. Also, payment will still be required if the child is absent for any reason (E.g., vacations on regular school days, sickness, etc.)
Expectations: 
1.
Drivers will wait no longer than 2 minutes for a student. If a student is not out of their home by that time, the driver must proceed to the next destination on their route. 
2.
Drivers will only make scheduled stops unless a non-schedule stop is previously authorized. 
3. Drivers will not text on their cell phones when driving students to and from school. 
4. Drivers will not allow students to eat or drink in the van (a $75.00 cleaning fee may apply per student if eating in the vehicle) 
5.
Suppose students act aggressively or unsafely (verbally or physically). In that case, the driver will pull the vehicle over, where appropriate, until students act safely and responsibly again. 
6.
Drivers will immediately (verbally) report all misbehavior to Mia Limos staff when they arrive at the office or by phone while driving students home. They will follow up by writing a formal bus report to be forwarded to parents within 24 hours of the incident. 
7. 
Students who continue acting aggressively after their parents are informed are subject to removal from the van, and no refund will be made.
On the Van: 
1.
Go directly to an available or assigned seat. 
2. Remain seated during the ride, wear your seat belt, and face forward. 
3.
Keep your head, hands, arms, and legs inside the vehicle. 
4.
Never play with emergency exit equipment. 
5. Never throw or pass around any object(s). 
6.
Never board the vehicle with live animals of any kind (unless permitted per driver). 
7.
Only carry-on items that can easily be held in your lap. 
8.
No eating or drinking. 
9.
No food or drinks in the Van.

Some of the vehicles will have audio and video recordings. This material will be shared regarding a specific incident involving your kid upon request.

VAN WAIVER (THIRD PARTY)

VAN TRANSPORTATION CONSENT AND RELEASET


I/We, am/are the parent and legal guardian of (“Student”). Student is enrolled in Greater Miami Hebrew Academy, Inc. (the “School”) for the 2023/2024 school year. By signing this waiver, I/We do so understanding the inherent risks and liabilities involved in the transportation of students. I/We hereby consent and give permission for Student to be transported to and from School by the following third party (“Third Party”): VAN VENDOR

I/We understand that the Third Party is unrelated to the School and is an independent third party. I/We further understand that the Third Party not an employee of the School and as such, the School does not supervise, train, or have any authority over the Third Party. I/We acknowledge that Student is being transported solely under the direction and control of the Third Party. I/We understand that I/we have an independent obligation to ensure that the Third Party, and the transportation provided is acceptable to me/us. I/we agree that the School has made no representations about the Third Party (including driving records, background, training, etc.), or any other matter that would have impacted my/our decision-making. I/we assume ALL RISKS inherent and otherwise, known and unknown, including, without limitation, risks of death, personal or bodily injury, disability or property damage resulting from, in connection with, or in any way related to the transportation of Student.

By signing below, I/we hereby, jointly and severally, and intending to legally bind myself/ourselves and Student agree to release, discharge, promise not to sue, and hold Greater Miami Hebrew Academy, Inc. and its predecessors, successors, and assigns, as well as all of their respective representatives, agents, owners, officers, directors, shareholders, managers, partners, employees, staff, volunteers, and supervisors, past and present, harmless from and indemnify them against all claims, demands, suits, charges, fees, attorneys’ fees, costs, damages, liens, liabilities, and actions of any kind whatsoever arising out or connected with, directly or indirectly, the transportation of Student. The types of claims I/we hereby release include contract claims, statutory claims, torts of any kind, negligence, intentional acts, economic and non-economic
damages, expenses, costs, insurance claims, attorneys’ fees, or any other type of claim.

I/We have signed this document knowingly, willingly, and after having an opportunity to consider its implications.

I/We understand that I/we do not have to sign this document.

School Van Registration

*** Submitting this form does not guarantee your seat on the van until you receive a confirmation email. Service operates on a first-come, first-served basis, and rates assume 14 children on the van; otherwise, rates may increase.

School Consent and Release

I agree to release and hold harmless the School, its agents, and employees from all claims, damages, or other liabilities for injuries to my child which are not the result of gross negligence by the School, its agents, or employees. I further agree to indemnify the School for damages to property by my child. I agree that the above-named student may participate in all School activities, including athletics, and any School-sponsored trip away from the School, unless the School receives written notices to the contrary. Although it is understood that Greater Miami Hebrew Academy and its representatives intend to take all reasonable precautions with respect to all activities, I understand that the participation of my child in the activities of the School programs involves a certain element of risk. I hereby assume that risk and agree to hold Greater Miami Hebrew Academy, Inc. and its employees, instructors, and representatives blameless from any liability unless the occurrence happens because of gross negligence or willful and wanton behavior on their parts. I will assume full responsibility for any personal injury that might occur to my child while taking part in School activities, and I will absolve and release the School and its employees and representatives from all liability and waive any and all claims in regard to such injury. The van registration of a student is considered an acceptance, on his/her part and on the part of his/her parents or guardians, of all rules and regulations of our School, including the judgement of School authorities on academic and disciplinary sanctions, suspension, or expulsion of a student from the van.
